Hypertrophic right L5 transverse process with pseudo articulation with the sacrum representing a type IIa lumbosacral transitional vertebra (Castellvi classification).

The facet joints are unremarkable. 

Case Discussion

Lumbosacral transitional vertebra (LSTV) is a commonly encountered finding. Its recognition is straightforward, but can be overlooked. Meanwhile, its clinical significance is in doubt. 

The presence of low back pain along with a lumbosacral transitional vertebra makes this syndrome, but it remains controversial.

According to Castellvi classification of lumbosacral vertebrae, this case is consistent with type IIa LSTV.
