Case contributed by Andrew Dixon
Diagnosis certain

Presentation

Car accident.

Patient Data

Age: 45
Gender: Female

Hypodense region within hepatic segment 7 with multiple foci of arteral contrast seen at the anterior and medial margin.  No subcapsular hematoma, perihepatic blood or other evidence of traumatic injury in this region.

Portal venous and delayed phase images show progressive centripetal contrast filling of the segment 7 abnormality characteristic of hepatic hemangioma.

Case Discussion

Example of a large hepatic haemangioma simulating a liver laceration with contrast extravasation in a trauma patient. Hemangiomas are common incidental findings and are important to consider as a differential for traumatic injury, particularly if there are no other supporting features of trauma in the region.
