Subarachnoid hemorrhage and left anterior cerebral artery aneurysm

Presentation

Presented to ER department with a sudden onset severe headache for 1 hour, frequent vomiting and syncopal episodes. Headache present for 5 days but deteriorated today. Drowsy with mild neck stiffness. Left sided weakness with motor power 4/5, positive Babinski sign on left side.

Gender: Male

CT brain with IV contrast showing saccular aneurysm at ACA. There is also subarachnoid space hemorrhage in non-contrast images.
