Subarachnoid hemorrhage - MCA aneurysm rupture

Presentation

Terrible headache of acute onset. GCS reduced.

Patient Data

Age: 40 years
Gender: Male

Diffuse sub-arachnoid hemorrhage throughout both cerebral hemispheres.

Large hematoma centered on the M1-M2 segment of the right middle cerebral artery. A 'filling defect' is visible within the clot, suggesting the culpable aneurysm. 

The basal cisterns contain blood as does the ventricular system.

Mild left temporal horn dilatation.

The location of the hematoma is highly suggestive of a MCA aneurysm, which was subsequently proven on catheter angiography.

The aneurysm underwent surgical clipping.
