Presentation

Hypoglycemic, prolonged (2 hours) seizures.

Patient Data

Age: Term baby

Marked diffusion restriction is seen throughout the cortex, sparing the basal ganglia and posterior fossa. 

Case Discussion

The pattern of brain injury in pediatric patients is complicated and varies considerably depending on duration, severity and underlying cause of insult. In the setting of pure hypoglycemic neonatal encephalopathy, the occipital cortex and thalami are particularly involved. In this case however, the hypoglycemia is combined with the changes from prolonged seizure which predominantly affects the supratentorial cortex.
