Orbital varix

Case contributed by Frank Gaillard
Diagnosis almost certain

Presentation

Enophthalmos.

Patient Data

Age: Adult
Gender: Male
ct

CT through the orbits obtained initially without contrast and then with contrast while the patient performed a Valsalva maneuver.

In the posterior and inferior aspect of the right globe a small soft tissue density region on the initial scan vividly enhances and markedly enlarges displacing the optic nerve superiorly and resulting in mild proptosis.

Features are characteristic of an orbital venous varix.

Case Discussion

The patient presented with paradoxical enophthalmos at rest because the globe falls back in the extra space created by distension of veins on straining.
