Patient Data

Age: 18 years
Gender: Male

Knee

mri
  • displaced fragments of articular cartilage with subchondral marrow edema and kinking involving the posterior half of medial femoral condyle and posterior third of medial tibial condyle articular surface. 
  • anterior translation of tibia with non visualization of femoral attachment of ACL. 
  • buckling of PCL.
  • Diffuse hyperintense signals within body of medial meniscus with complex tear of posterior horn. 
  • positive fragment in notch sign. 
  • moderate knee joint effusion with fluid in retropatellar bursa, prepatellar bursae. 

Case Discussion

Macrotrauma to the articular surface can result in fragmentation, displacement and subchondral injury as demonstrated in the medial compartment of the knee. An accompanying ACL tear and complex medial meniscal tear is seen with moderate reactive effusion.
