Testicular torsion

Edematous right testis without internal flow. Twisted intra-scrotal right spermatic cord.
Normal left testis with vascularity.
Case Discussion
Ultrasound shows a primary sign of torsion (twisted spermatic cord) and the secondary signs of torsion (edematous, infarcted testis).
Surgery (within 2 hrs) confirmed the presence of both primary and secondary signs of torsion. There were two complete turns of cord. Testis was not viable.
