Presentation
This is a 55-years-old male patient with positive family history of lung fibrosis presented with shortness of breath and chronic cough.
Patient Data
Diffuse bilateral reticular shadowing with lower lobe predominant honeycombing. Minimal decrease in both lung volumes with irregular heart border (shaggy heart border) seen. Findings are suggestive of fibrosis.
CT confirms the changes of fibrosis and is showing diffuse bilateral reticulonodular shadowing with lower lobe predominant honeycombing and focal areas of traction bronchiectasis as well as interlobular septal thickening.
Lytic expansile bony lesion is seen within the first right rib consistent with benign lesion. This was an incidental finding.
Case Discussion
The constellation of CT findings (i.e. the lower lobe predominant honeycombing), patient presentation as well as patient's family history are concerning for idiopathic pulmonary fibrosis (usual interstitial pneumonia).
