The imaging findings were initially attributed to a primary demyelinating process such as multiple sclerosis.

The patient had a longstanding history of deafness and vision impairment. The diagnosis Susac syndrome is made on the clinical triad of:

• vision impairment
• deafness
• encephalopathy
