Polymicrogyria and heterotopia

Patient Data

Age: 20 years
Gender: Female

Extensive grey matter heterotopia and polymicrogyria demonstrated in the right temporal extending into the right occipital lobe with involvement of the right lateral ventricle trigone. The septum pellucidum is present and optic chiasm/nerves appear normal. A small cleft extend laterally from the trigone of the right lateral ventricle suggestive of a closed lip schizencephaly. 

The right hippocampus and amygdala are enlarged and demonstrate loss of normal architecture and are dysplastic.  Susceptibility present in the right frontal lobe may represent a small telangiectasia, or small low flow arteriovenous malformation. The reminder of the study is normal.

Case Discussion

Closed lip schizencephaly can be difficult to detect, and may well be present in this case.
