What is the diagnosis? A. Air B. Dermoid cyst C. Epidermoid cyst D. Inflammatory cyst
Dermoid cyst. There are multiple hypodensities but they follow fatty attenuation. Of the choices, dermoid cysts are only lesions that follow fat density. Epidermoid and inflammatory cysts are expected to behave closer to CSF.
Multiple sequences through the brain demonstrate disseminated fatty droplets within the subarachnoid space, compatible with a ruptured dermoid.
