Diagnosis almost certain

Presentation

Known diabetic, hypertensive with recent onset of unilateral eyelid swelling, mild proptosis and restriction of ocular movements.

Patient Data

Age: 65 years
Gender: Female
ct

Dirtying of right retrobulbar orbital fat with involvement of the anterior attachments of superior and lateral recti and also surrounding a mildly enlarged lacrimal gland. The other extraocular muscles, optic nerve, orbital apex, ophthalmic artery are normal. Associated preseptal edema present. 

Case Discussion

Idiopathic inflammation of the orbit is commonly encountered with involvement of extraocular muscles, distinguished from thyroid ophthalmopathy by the involvement of tendinous insertions of muscles which is typically spared in the latter.
