Loss of normal signal intensity and fibers continuity of the ACL fibers suggesting it is torn.
Shortening and abnormal oblique linear hyperintense signal is seen within the posterior horn of the medial meniscus disrupting its articular surface representing a meniscal tear. The detached fragment of the posterior horn is seen parallel and anteroinferior to the PCL giving rise to a double PCL sign suggesting a bucket handle tear.
Case Discussion
The double posterior cruciate ligament (PCL) sign is a low-signal-intensity band that is parallel and anteroinferior to the PCL on sagittal MR images.
It is a highly specific indicator of a bucket-handle meniscal tear.
