Presentation
Sudden onset pleuritic chest pain and dyspnea. History of unprovoked pulmonary emboli. Non-compliant with warfarin.
Patient Data
Small right sided pleural effusion is present with a wedge shaped infiltrate in the right middle zone.
There is a large right sided pulmonary embolus occupying the distal part of the right pulmonary artery. There is a small right sided pleural effusion with right sided peripheral subpleural changes consistent with pulmonary infarcts. No evidence of right heart strain or pulmonary hypertension.
Case Discussion
This is a case of a large pulmonary embolus with associated pulmonary infarcts. The patient had a history of unprovoked pulmonary emboli and had been non-compliant with his warfarin.
