Anterior neck swelling.
Patient Data
A well-defined, thin-walled, fluid-filled cystic lesion is noted deep to platysma at the right side of the neck, intimately beneath the right strap muscles and resting on the right lamina of thyroid cartilage. It wraps around the posterior and inferior surfaces of the hyoid bone with extension into the pre-epiglottic space. It showed minimal wall enhancement.
Multiple upper deep cervical inflammatory lymph nodes are noted.
Case Discussion
Thyroglossal duct cysts are an anterior midline cystic swelling along the course of thyroglossal duct, more common to be infrahyoid and can be a little off midline. Thyroglossal cysts may rarely extend into the laryngeal pre-epiglottic space as it curves backwards and superiorly to reach behind the hyoid bone, corresponding to the course of thyroglossal duct.
