Presentation

Ongoing left mid-calf pain, aggravating at night. No prior medical or surgical history. Referred by GP.

Patient Data

Age: 20 years
Gender: Male
x-ray

Extensive cortical thickening and reactive sclerosis involving medial aspect of the distal diaphysis of the tibia.

There is an impression of a small central cortical lucency suggestive of a tiny nidus.

Nuclear medicine

Intense delayed focal uptake seen in the medial left mid-tibial cortex correlating with the thick periosteal reaction on the radiographs.

At the epicenter of the increased uptake lies a 5 mm photopenic lesion located in the medial tibial cortex consistent with nidus of an osteoid osteoma. 

Case Discussion

Osteoid osteoma is benign tumor of the bone which typically causes nocturnal pain, classically alleviated by aspirin. 

It shows typical focal intense radiotracer uptake on a bone scan. 

This patient was successfully treated with radiofrequency ablation of the lesion.
