Hepatic adenoma

Case contributed by Amr Farouk
Diagnosis almost certain

Presentation

Focal hepatic lesion discovered on sonography.

Patient Data

Age: 50 years
Gender: Female
mri

Non-cirrhotic liver showing a right hepatic lobe segment VIII focal lesion eliciting subtle isointense signal on T1 WI, mildly hyperintense signal on T2 WI, and on the dynamic post-contrast sequence, it showed early arterial enhancement becoming nearly isointense about liver on delayed images.

Case Discussion

A right hepatic lobe focal lesion in a female patient with long history of oral contraceptive pills (on inquiring) and non-cirrhotic liver in addition to the dynamic MRI findings of early arterial enhancement becoming nearly iso-intense about liver on delayed images with no washout of contrast makes hepatic adenoma the most likely diagnosis.

The right hepatic lobe focal lesion was subsequently pathologically proven as an adenoma.
