What is the term used for involvement of the entire cord?
Holocord. The word comes from greet "holo-" meaning "whole" or "entire" and not from hollow, as is sometimes thought to be the case.
Given the very large flow voids, and solid enhancing mass, what should be your favoured diagnosis? What would be your differential?
A hemangioblastoma should be favoured, although an ependymoma and vascular metastasis (e.g. renal cell carcinoma) should also be considered.
In the lower thoracic cord is a vividly enhancing nodule with associated prominent serpiginous flow voids both within the mass and over the surface of the cord, and among the cauda equina. It is an isolated abnormality, associated with marked expansion of the entire cord by a large syrinx.
