Hip pain.
Patient Data
On the right, the femoral head is sclerotic, partially collapsed and with an irregular cortical surface. Mild joint space narrowing superiorly. On the left, there is femoral head subchondral lucency with less severe sclerosis. There is some articular surface irregularity.
Bilateral avascular necrosis characterized by subchondral serpiginous signal, worse on the right. There is partial collapse of the right femoral head with secondary osteoarthritis, joint effusion and full-thickness tear of the acetabular labrum. On the left established avascular necrosis without collapse nor established osteoarthritis.
Case Discussion
The underlying cause is not known but is likely related to treatment to the patient's known history of Fanconi anemia.
