CT through the left temporal bone demonstrates a circumscribed region of demineralization immediately anterior to the oval window, in the bone between the cochlea and vestibule.

The stapes' footplate fills the oval window. Immediately anterior to it is a region of lucency. 

Case Discussion

This is a characteristic appearance for fenestral otosclerosis. It is a subtle finding which is often bilateral. As such getting used to the normal appearance of this region is essential if the diagnosis is to be made.
