Pleomorphic adenoma - large recurrence

Diagnosis almost certain

Lobulated mass in the parapharyngeal space / deep lobe of parotid widens the stylomandibular interval. 

In the left parapharyngeal space / deep lobe of parotid is a large lobulated mass with high T2 signal, intermediate T1 signal and vivid contrast enhancement. It displaces the mucosal space medially and results in obstruction to drainage of the left middle ear, which is opacified by fluid.

Post left parotid region surgery, with faty atrophy of the gland demonstrates a small enhancing nodule in the deep lobe of the parotid gland.

Case Discussion

This patient had had a pleomorphic adenoma resected a few years earlier. This is a large recurrence. A well described and particularly troubling complication.
