What is a possible diagnosis based upon MRI findings in this patient with ataxia, vertical gaze palsy and amnestic syndrome, who presents 2 months after bariatric surgery (sleeve gastrectomy)?
Possible Wernicke encephalopathy. Blood samples were taken for laboratory testing of nutritional deficits (thiamine and folate). Treatment with thiamine was started on the same day on spec.
- Periaqueductal T2 hyperintensity involving the quadrigeminal plate.
- T2 hyperintensity of the mammillary bodies.
No restricted diffusion. No contrast enhancement. No signs of infarction or inflammation.
