Free-floating thrombus in the internal carotid artery

Discussion:

There is a small projection of intraluminal thrombus at the right ICA. This has been described as "free floating thrombus", an uncommon finding placing this patient at high risk for a more devastating ischemic event. This is typically managed surgically with carotid endarterectomy or medically with anticoagulation therapy.
