Carbon monoxide poisoning at her workplace five years ago, she underwent treatment then. Now complaining of headaches.

Patient Data

Age: 55 years
Gender: Female

MRI

mri

There are symmetric T2-hyperintense lesions in globi pallidi bilaterally. These regions also demonstrate subtle T1-hypointensity, best appreciated on axial views.

Case Discussion

Given this patient had a known history of carbon monoxide poisoning, these radiographic changes, which have characteristic symmetric T1 and T2 signal changes within the globi pallidi, are almost certainly due to the carbon monoxide poisoning.

In a patient without such a known history, other etiologies for bilateral basal ganglia lesions should be considered, including other toxic encephalopathies.
