Splenic flexure cancer with omental caking

Diagnosis almost certain

Presentation

General fatigue and loss of weight over last 6 months. Iron deficiency anemia.

Patient Data

Age: 76
Gender: Female

Extensive omental soft tissue infiltrate is present in the left abdomen. A stenosing mass is also noted in the splenic flexure, with no upstream colonic distension.

Further peritoneal nodules are demonstrated adjacent to the liver, indenting the dome and just posterior to the caudate. No definite hepatic parenchymal lesions.

Conclusion:

Splenic flexure mass is suspicious for carcinoma, with extensive omental caking and peritoneal metastases.

Confirmed at surgery.

Colorectal cancer is known to metastases to the peritoneum. One manisfestation of peritoneal carcinomatosis is omental caking, which is seen in this case.
