This is a classic appearance of necrotizing soft tissue infection of the scrotum, most common in diabetic patients. This is managed with aggressive surgical debridement, as in this case.

Culture results:
Enterococcus faecium - numerous growth
Streptococcus, viridans group - moderate growth
