Intracranial lipoma

A 12 mm lesion centered in the pineal region and extending into the left ambient cistern is composed of fat (HU -68) located above the pineal and habenular calcifications. These features are those of an intracranial lipoma. It is extremely unlikely that this represents another process (e.g. teratoma) are considered unrealistic differentials.
Case Discussion
Intracranial lipomas are relatively common incidental findings that are usually completely asymptomatic and generally do not require additional imaging or followup.
