Case contributed by Henry Knipe
Diagnosis probable

Presentation

Follow-up.

Patient Data

Age: 30 years
Gender: Female

There is a 5 x 5 x 5cm splenic cyst with internal echogenicity suggestive of particulate matter with a relatively thick echogenic region inferiorly. There is no internal vascularity on Doppler. No splenomeglay.

6 years prior

ultrasound

Hypoechoic splenic lesion measuring 5.1 x 3.9 cm with central echogenic foci.

11 years prior

ultrasound

Hypoechoic splenic lesion measuring 2.9 cm without internal echogenicity. No Doppler signal.

Case Discussion

The patient was initially evaluated for right upper quadrant pain and the splenic lesion was an incidental finding. It has slowly grown over 11 years and has become more "complex" with internal echogenic foci developing secondary to internal debris. These features are fairly typical of a splenic epidermoid cyst.
