Anterior shoulder dislocation

Case contributed by Andrew Murphy
Diagnosis almost certain

Presentation

Patient with a history of recurrent anterior glenohumeral dislocations. Presented today after falling from a standing height.

Patient Data

Age: 60 years
Gender: Male
x-ray

There is an anterior-inferior dislocation of the glenohumeral joint, Additionally there is an engaged Hill-Sach lesion evident on the Garth's projection.

Case Discussion

Large Hill-Sach lesions that are also engaging are correlated to recurrent shoulder instability 1

The Garth's projection in this instance is an ideal radiographic projection to examine the relationship of the humeral head and the glenoid, the projection requires no abduction and little to no patient discomfort.
