Presentation
Intoxicated and intubated. CPR performed. Is there a rib fracture?
Patient Data
There is right upper lobe collapse, with an endotracheal tube observed obstructing a tracheal (pig) bronchus. Ill-defined opacification demonstrated in the right lower zone may relate to aspiration pneumonia and/or a pleural effusion.
Following retraction of the endotracheal tube, collapse of the right upper lobe has resolved. This plain radiograph was taken one day after the previous radiograph.
Case Discussion
This case illustrates the principle of collapse due to a malpositioned endotracheal tube. In this case, a tracheal bronchus was obstructed, with collapse quickly resolving following resolution.
