This case demonstrates typical appearances of a medulloblastoma arising from the roof of the fourth ventricle with prominent leptomeningeal spread.

Restricted diffusion, extensive supratentorial leptomeningeal spread, apparent origin from the roof of the ventricle and lack of soft extrusion through the foramen of Luschka and Magendie, all suggest this is not an ependymoma.
