Presentation
Sudden vision loss of the right eye with papilledema on the ophthalmological exam. The patient describes the same history on the left side 4 years ago.
Patient Data
Swelling with high signal intensity and contrast enhancement of the retrobulbar intra-orbital segment of the right optic nerve. The left optic nerve appears atrophied mainly at its proximal and mid-portions (diameter = 1.3 mm). No demyelinating lesion was seen at the infra- or supratentorial level.
No spinal cord lesion is seen.
Case Discussion
MRI appearances of right optic neuritis with contralateral optic nerve atrophy (Pseudo-Foster Kennedy syndrome).
The unilateral optic neuritis, the absence of a spinal cord lesion as well as the clinical presentation make neuromyelitis optica (NMO) much less likely.
