There is a well-defined mass arising from the sphenoid sinus, extending through the sphenoethmoidal recess to the nasopharynx, obstructing both choanae (mainly the right). It appears of low signal intensity on T1WI, high signal intensity on T2WI with peripheral enhancement following IV contrast.

Case Discussion

 Sphenochoanal polyp is a rare benign tumor, arising from the sphenoid sinus. The main symptom is a progressive nasal obstruction. The differential diagnosis should include the antrochoanal polyp which is the most common. The treatment is surgical (usually endoscopic approach).
