Anterior cruciate ligament complete tear with posterior cruciate ligament partial tear and posterolateral corner injury

Presentation

The patient presented after knee trauma with knee swelling and pain.

Patient Data

Age: 30 years
Gender: Female

Non-visualization of the ACL fibers indicative of complete tear.

The PCL is swollen (measuring about 10 mm in maximum thickness ) with high signal intensity in T2WI compatible with a partial tear.

Posterolateral corner injury manifested by:

  • Torn fibular collateral ligament and bicipes femoris tendon insertion.
  • Torn popliteus tendon.
  • Avulsion fracture of the fibular styloid process (arcuate fracture).
  • Sprain of the popliteus muscle.

Grade I injury of the MCL.

Sprain of the medial and lateral heads of the gastrocnemius muscle.

Diffuse extensive subcutaneous and interstitial edema.

Minimal joint effusion with pre & infra-patellar bursitis.
