Presentation

Dementia and gait disturbances for 5 months

Patient Data

Age: 50 years
Gender: Male
mri

Bilateral confluent symmetrical T2/FLAIR  hyper intensities noted in deep cerebral white matter associated with mild cerebral atrophy. No mass effect and No diffusion restriction seen.

Note made of bilateral mastoiditis.

Case Discussion

As the patient is known HIV+ with bilateral extensive cerebral white matter changes and atrophy, diagnosis of HIV encephalopathy was made.

Differential diagnosis is progressive multifocal leukoencephalopathy (PML) where there will be involvement of subcortical U-fibers.
