The MRI sequences demonstrate a right frontoinsular mass of complex signal intensity. Centrally, it elicits an isoisignal to the cortical grey matter on T1, high signal on FLAIR, and T2 with a peripheral component of low signal on T1 and high signal on FLAIR with a fluid-blood level on T2. Surrounding oedema is noted with a mass effect on the midline structures with subafalcine herniation.
The DWI shows high signal centrally with low ADC (restricted diffusion) and low signal peripherally with high ADC, due to a reduction in the extracellular space and increased viscosity.
No evidence of arteriovenous malformation (AVM) or aneurysm is seen.
