Patient Data
There is inferior and inferolateral wall aneurysm of the left ventricle. Dilated left ventricle is also demonstrated.
There is hyperintense focus at the aneurysm wall on late contrast enhancement images indicative of myocardial fibrosis (yellow arrows).

Case Discussion
This patient has decreased LV ejection fraction, caused by dilated cardiomyopathy, confirmed with echocardiography. Also, there is notable decrease of ventricular contractility on short axis CINE images, dilatation of left ventricle, left ventricular aneurysm and fibrosis of the aneurysmal wall.
