Mildly elevated lipase.
Patient Data
The pancreas has equivocally enlarged appearances with loss of the pancreatic clefts. No peripancreatic fat stranding or fluid. The main pancreatic duct is not dilated. The remainder of the abdominal viscera is normal.
The pancreas is enlarged with loss of its clefts, shows delayed enhancement in the multiphase post-contrast sequences, and has marked restricted diffusion.
Case Discussion
This patient had elevated IgG4 levels. Further imaging follow-up with MRI performed 6 months later reviewed normal pancreas (images not shown).
The imaging and clinical picture are those of autoimmune pancreatitis.
