Hamstring origin complex avulsion

Presentation

Fall from height onto leg.

Patient Data

Age: 70 years
Gender: Female

Avulsion of the common hamstring origin, involving semimembranosus, semitendinosus and the long head of biceps femoris with fluid in the defect, which measures 2.9 cm.

Intermediate signal and thickening of the redundant semimembranosus and biceps femoris fibers, over a length of 3.3 cm.

Fascial plane edema tracks caudally. Minor perineural edema in relation to the sciatic nerve.

Fatty atrophy of the proximal semitendinosus muscle belly in keeping with a prior injury. 

Case Discussion

This is an example of hamstring origin complex avulsion, which in adults tends to be tendinous cf. adolescents where the avulsion injury involves the apophysis. This would be given a BAC grade 4.
