The gallbladder is distended containing multiple gallstones and sludge, and one of the stones is lodged in the gallbladder neck. It has a thickened and enhancing wall. There is an enhancement of the adjacent liver parenchyma (reactive hyperaemia). The combination of findings is compatible with acute cholecystitis.
The impacted gallstone in the gallbladder neck compresses the common hepatic duct (CHD) with mild dilatation of the intrahepatic biliary ducts.
