Presentation

Swelling of both calves, pain and difficulty in walking.

Patient Data

Age: 25 years
Gender: Female

Gross fatty atrophy of bilateral soleus muscles with near complete fatty replacement. Moderate to severe fatty atrophy of bilateral medial heads of gastrocnemius muscles is seen (left>right). Bilateral lateral heads of gastrocnemius muscles are spared. Mild fatty atrophy with edema in bilateral tibialis posterior muscles is seen (right>left).

Significant fatty atrophy of muscles of posterior compartments of thigh bilaterally including adductor muscles.

Case Discussion

Above findings are consistent with muscular dystrophy. The pattern and groups of muscles involved suggest the possibility of limb girdle muscular dystrophy (LGMD2A).
