Enlarged spleen with a large well-defined multiseptated cystic lesion with posterior acoustic enhancement.

The CT images show homogeneous hypodense fluid content of the splenic lesion with small peripheral calcification. The postcontrast images show a slight enhancement of the lesion's septations with no solid component seen.
Case Discussion
Ultrasound and CT features are suggestive of a splenic lymphangioma. Large lymphangioma may be symptomatic as in this case due to the enlarged spleen with a mass effect 1. The main differential diagnosis in this case, is a hydatid cyst.
