Posterior cruciate ligament rupture

Presentation

Four weeks post football injury.

Patient Data

Age: 25 years
Gender: Male

Moderate size knee joint effusion.

Medial meniscus is intact. Discoid lateral meniscus occupying nearly 100% of the articular surface; no focal tear. Similar-sized anterior and posterior horns. Meniscofemoral ligament and posterior meniscocapsular attachments are intact.

No parameniscal cyst formation or meniscal body extrusion.

Anterior cruciate ligament is intact. Full-thickness mid-fiber posterior cruciate ligament rupture. Medial collateral ligament is intact. Minimal high signal of the lateral collateral ligament origin. Popliteus muscle belly edema.

Case Discussion

This case demonstrates a full-thickness (grade 3) posterior cruciate ligament rupture with associated popliteus muscle injury and a grade 2​ proximal lateral collateral ligament sprain.

A discoid lateral meniscus was incidentally identified but was not injured.
