Aortic coarctation with aneurysm

Case contributed by Michael P Hartung
Diagnosis certain

Presentation

Abnormal echocardiography with aneurysm.

Patient Data

Age: 20 years
Gender: Male

Large aneurysm of the aortic root.

Some fluid and stranding in the mediastinum.

Coarctation with severe focal narrowing of the distal arch beyond the left subclavian artery.

Enlarged great vessels with numerous paraspinal and internal mammary collaterals.

Case Discussion

Dramatic case of both aneurysm and coarctation of the thoracic aorta.

Collateralization is through the brachiocephalic, left subclavian, and internal mammary arteries.

Fluid in the mediastinum raises the concern of possible early hemorrhage or rupture, or perhaps is simply related to venous/lymphatic congestion.
