Why is it important to detect an injury to the spleen?
Because trauma to it can be associated with significant blood loss and is life threatening.
In what context do you suspect splenic injury?
In trauma to the lower left thorax or to the upper left abdomen, be it penetrating, blunt or indirect.
Moderate splenomegaly. There is an ill-defined hypodense peripheral area along the superior lateral aspect of the spleen, consistent with a grade 2 AAST splenic injury.
There is no active contrast extravasation.
There is a tiny adjacent subcapsular splenic hematoma.
Hematoma in the right thigh not completely visualized, with no active contrast extravasation.
