Lower abdominal pain, ultrasound revealed adnexal lesion
Patient Data
Right ovarian cystic lesion showing areas of fatty component (T1 & T2 high signal, with corresponding signal suppression in fat sat sequences) floating over fluid component (T1 low & T2 high signal). Also, foci of signal void in all pulse sequences noted in the inferior part of the lesion denoting calcification. No post contrast enhancement noted.
No related edematous changes. No signs of torsion in this examination.
Note that the left ovary is bulky with multiple small follicles and no dominant follicle seen.
Case Discussion
This is a typical appearance of ovarian dermoid cyst, showing fat and calcification.
Considering the patient history of lower abdominal pain and the imaging features of this lesion being large in size and located in the mid-line, one's mind should be directed to exclude ovarian torsion. However, torsion/detorsion is still a possibility.
