Describe your findings
Evidence of a large knee joint effusion with layering fluid of different echogenicities in the suprapatellar pouch. The most superficial of these fluids is high echogenicity, consistent with fat. Findings represent a lipohaemarthrosis. The patella and quadriceps tendons are intact.
Would you suggest any further imaging?
The presence of a lipohaemarthrosis signifies an intra-articular injury and CT is the most appropriate next investigation. A radiograph could be considered if not already performed, although the patient will likely need a CT regardless.
