Two day right flank pain with high fever (39.4) in pregnant women (29 week). Laboratory analysis: WBC 20,200/mm3 ; CRP 302,6. Biochemistry urinalysis: cloudy; pH 5.0; spec. gravity 1.005; protein +; nitrite +; blood +; RBC 3-5; WBC 10-30; many bacteria.

Focal patchy echogenicity area in upper pole of the right kidney parenchyma, with loss of corticomedullary differentiation and hypoperfusion on color Doppler signal.
Case Discussion
Ultrasound examination revealed focal pyelonephritis which was confirmed by clinical and laboratory findings.
