[This sample is from a core biopsy of a lung metastasis]
The original thyroid FNA showed highly cellular specimen with anaplastic features. She was otherwise feeling well at time of procedure but that night returned to the hospital via the emergency room with difficulty breathing. Imaging showed multiple lung masses and bilateral adrenal masses. Lung FNA was identical to the thyroid.
This is the lung core biopsy. The tumor is strongly positive for PAX-8, pankeratin (AE1-AE3), and vimentin. It is negative for SOX-10, thyroglobulin, calcitonin, and synaptophysin. EMA is weakly, focally positive.
Diagnosis: Undifferentiated carcinoma, consistent with metastasis from thyroid primary.
